From what I read years ago, he didn't stretch it, it just grew that way as did Long Dong Silver's. There's a vid of LDS walking and taking a leak on this site. You should see the look on the face of the man he's taking a leak next to.
A classic! I wish that the original photo had been clearer, but all the versions of it that I have seen have been fuzzy like this, so there's no way to know if it is a genuine photograph or a repainted one.
