The blonde is a visiting nurse (hence the white coat) tending to Ian Scott after a household accident. While she pays lavish attention to his monster, then takes it up both holes, what sets this scene apart from the same-old/same-old is the money shot--a creamy compromise between swallowing his load (thus hiding it from view) and having it sprayed all over her face or backside--which is, after all, not what usually happens--except in porn. What we see here is far more realistic and (I think) hot-looking.
